Qualifications

Required:

Associate degree or higher
1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
ANY of the following valid licenses/certifications:
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Basic Life Support (AHA BLS)
Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all three)?

Preferred:

Overview

Our Award Winning Client is seeking a compassionate Home Health LPN to join their extraordinary care team in Greeley/Lords Valley, PA . The ideal candidate will have an Associate degree, an active LPN license in Pennsylvania, and two years of LPN experience . Responsibilities include collaborating on patient care plans, supporting patients in the local area, and utilizing technology tools effectively.

Key Responsibilities

Collaborate to manage and deliver patient care plans
Support patients in Greeley, Lords Valley, and surrounding areas
Meet critical deadlines for charting
Self-manage time and schedule efficiently
Demonstrate experience with tablets, mobile phones, and EMR software

Schedule and Shift Details

Schedule: Monday-Friday, 8 AM - 5 PM with one weekend on-call every 4-8 weeks , depending on staffing.
Weekend On-Call Details: On-call for Admissions only on Saturday & Sunday from 8 AM - 5 PM (if needed). To compensate, two weekdays off will be provided.

Home Health LPN – $10,000 Bonus

Location: Lords Valley, PA
Territory/Driving Expectation: Pike County
Preferred Experience: Home Health/Hospice, Acute Care, LTC, SNF
Not Preferred: N/A
Pay Rate/Salary: $46-$54 per visit with a $10,000 FYS bonus
