Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Sacramento, California. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Sacramento.
As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.
This full-time Clinical Evaluator position offers both flexibility and purpose—combining remote work from your home office with local travel. Acentra Health covers and reimburses all travel-related expenses, including mileage and parking. Additionally, we provide all necessary technology and equipment for your home office setup, including a laptop for use while in the field.
Job Responsibilities
Conduct PASRR screenings, evaluations, and determinations through comprehensive clinical assessments of patients, including review of relevant records and input from collateral sources.
Required Qualifications, Knowledge, and Experience
Must possess an active, unrestricted State of California Licensure in a healthcare or related field, such as Licensed Psychologist, Licensed Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker, Social Worker Independent Clinical License, Nurse Practitioner, or Psychologist.
Graduation from an accredited college or university with a degree in a relevant field (e.g., psychology, social work, counseling, or related discipline).
Minimum of 2+ years of experience conducting assessments for individuals with mental health or psychiatric disorders, including complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia.
At least 1+ year of experience working with individuals with intellectual or developmental disabilities or related conditions.
Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.
Working knowledge and understanding of diagnosing psychiatric disorders in accordance with the most current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) applicable in the state. .

Compensation
The compensation for this role is $85,000.00 to $89,000.00 base salary
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
